FRIDAY ONLY: FROM 10:00AM-2:00PM

All replacement request must come into the Fairfield Facility.  FRIDAY ONLY  from 10:00am-2:00pm, you may come to our location for your replacement certifications.  You must arrive before 2:00pm.  Please be sure to have the neccessary information to research our records.  Name of course taken, name of certificates needed, date of course, location or name of instructor.  The more complete/correct information will faciliate the process.  Each replacement card is $25.00.  We can not issue certificates that are expired.
